President’s Council approved the Confidentiality of Student Information Policy in March 2015 after extensive discussion through the Administrative Policy Committee.
This Policy sets out standards for the confidentiality and access of Student Information to better protect student privacy in accordance with the Ontario Freedom of Information and Protection of Privacy Act (“FIPPA”) and the Personal Health Information Protection Act (“PHIPA”).

Highlights:
- The Informal Request for Student Information Form REPLACES the former Authorization to Release Student Information Form; the new form requires a notary to confirm the signature of the student when the form is sent remotely; students, when presented photo identification in person, may continue to sign with a witness when completing the form in person. The NEW form is to be used to support any requests from a third party for student information, including those requests that are received from external legal counsel.
